Use the Illinois search
IDFPR, the Illinois licensing agency, has a free search for doctors, nurses, physician assistants, pharmacists and pharmacies.
Its licence form uses “MEDICAL BOARD” for doctors, and “PHARMACY” for pharmacy records.
Use the licence type that matches the person. Compare the name and licence number with the details you received.

The Illinois form lets you filter for active licences, and IDFPR publishes separate reports of disciplinary actions.
Discipline is action the state takes over a licence holder’s conduct. Check whether the licence is active, then look separately for action over past conduct.
Source AA doctor’s profile adds context
As of September 2026, Illinois public doctor profiles show final action over a doctor’s conduct from the most recent ten years.
They also show money paid after claims that a doctor’s care caused harm, from the most recent ten years, as of September 2026.
A payment to settle a claim is not, by itself, proof that the doctor gave bad care.
These profiles cover doctors, not every kind of clinician.
Source BCheck a pharmacy that ships here
An out-of-state pharmacy mailing prescriptions to an Illinois resident needs an Illinois nonresident pharmacy licence, as of September 2026.
Nonresident means the pharmacy is based outside Illinois. Ask it for its Illinois registration details.
If a record is hard to find, ask IDFPR through its official site how that licence is listed. Do not guess from a similar name.
